My symptoms are very similar to yours - I could go for hours at a time and not even really know I had to go, or I could just as often urinate several times in a short period. Generally, it was common for me to void anywhere from 12 - 18 times a day. Ultimately, I was diagnosed with neurogenic bladder and they threw in OAB and urge incontinence for good measure! Testing showed I was retaining 400-500 cc after voiding also.
At the time, I had control most of the time but had "accidents" and also wet the bed nightly. I had TUIP surgery to open the internal sphincter valve in the hope that it would allow my bladder to drain more completely. While the incontinence was a concern, the bigger issue was the increased risk of UTI and bladder infection due to significant retention, so I had the surgery in Nov. 2017. Immediately following the surgery, I essentially lost all control (well, I could hold it for maybe 10 - 15 seconds). At this point, I can have anything from a sudden urge followed by a flood, to no feeling/warning, etc. I can't usually tell when my bladder is full.
Over the past month, I have been trying timed voiding, which is showing some promise. I resisted trying it because I haven't read much about positive results, but my wife convinced me to give it a shot. Basically, I have been trying to make myself go about every hour or so. It doesn't always work, but I have been able to go without a diaper for a while around the house, since I am close to a bathroom. It hasn't done anything to change my nighttime issues, and I'm not sure I will ever be at a point where I don't need protection, but depending on how it goes, I hope to one day be able to go without a diaper if there is a restroom handy. It may be unrealistic, but it gives me a little hope!
