Follow up incontinence survey

Hello,

This survey is a follow up supplementary survey to the one I posted a few weeks ago. It addresses questions not answered on the previous survey such as income level of those who don't receive diapers from medicaid or other insurance, whether one would be willing to participate in fundraising to support the organization in exchange for receiving products, and other questions. Some questions will most likely not pertain to all, and that is fine. Simply fill the potions that do pertain to you. Rest assured that even though this survey is asking personal questions about income, responses are 100 percent anonymous.
