This happened to me, but in public. I was getting on a bus to ride from the parking lot to my shop. I realized that my shirt had slid up, and the top of the diaper was sticking out in plain view. I had noticed one lady was looking at me with a surprised look. So, I knew I had outed myself. After 20 years of wearing diapers, I stopped caring a long time ago, though. All my family, friends, and co-workers know about my bladder control issues, anyways.

Years ago .. when the Depend Brand of adult diapers were green in color .. I personally petitioned Kimberly Clark to change the color from green to PLAIN WHITE. For this reason.. I shy away from the purple-in-color Molicare products. Of course I no longer use their products anyway with Abena and the other premium products so much more superior.

I too was concerned about this so recently invested in a number of proper onsie/body stockings that I wear when going out. This helps me remove some of the shame and potential embarrassment that I have at the moment with wearing diapers. If cost is an issue a leotard does the job just as well, I chose this option at first (as an experiment) in the UK we have a wide range of Charity shops and the leotard I found only cost a couple of pounds an absolute bargin.

For years, I found my wife checking to make sure my plastic pants were not exposed. I do not remember when but a long time ago she bought me 6 pairs of onesies. I rebelled at first but she convinced me to give them a try. They work well as well as help keep very wet diapers in place.
I do not recall where she bought them but for the last few years we have been buying them from "baby-pants.com". They are geared to the AB/DL set but their onesies are extremely well made and last a long time. Yes, they are a bit expensive but they fit good and really do the job. They have them in colors but I purchase plain old white.

I use the Gary activewear PUL plastic pants in dark colors. They hide the top of the diaper. If the top of the dark Gary PUL pants happens to show, most people will not recognize it as diaper-related and will just think it is the top of dark underwear.

I use them from baby-pants.com too. They work very well and hold together great. I like the options for some of the colors instead of just plain white all the time. I have a blue one that I can wear to work when I wear my dark blue polo shirt on Friday's. It doesn't show too much and if it does people think I am wearing a blue t-shirt under the polo. I need something under that and under my belt. Just the way I wear clothes these days.
